Care Management Advocacy Project (CMAP)

The Care Management Advocacy Project (CMAP) provides advocacy support to Disabled people living in Bristol who are having an Adult Community Care or Mental Health Service Assessment.
Our Advocacy worker will;
- Help you to express your needs and say what you want.
- Ensure you understand what is happening, so that you can make informed decisions.
- Support you to communicate with the Social Worker or other person assessing you.
Why do people need advocacy?It is important that you understand what is happening in the assessment process and that the people making decisions about your care needs have all the information they need. It is important that you are able to express your needs fully and that you are fully listened to and understood.
Sometimes the assessment process can be confusing or even intimidating. Having an advocate work with you means that you have someone on your side throughout the process, helping you to feel empowered and in control during your assessment.
Information for Social Workers and other Practitioners
If you are a social worker or other practitioner undertaking a care management or service assessment, the involvement of an Advocate can help the service user to communicate more clearly with you. This can enable you to make more accurate assessments and reduce the need for time-consuming reassessments.
